This Privacy Policy explains how U-Prime Wellness and its U-Prime Care service collect, use, store and share personal information through uprimewellness.com, online orders, appointment bookings and related communications. In this policy, “U-Prime” means U-Prime Wellness and U-Prime Care.

Information we collect

U-Prime may collect:

• identity and contact information, including name, email address, telephone number and delivery or billing address
• appointment information, including the selected service, healthcare provider, appointment time and booking status
• patient information, including date of birth, calculated age, sex, ethnicity, optional blood group, reason for the consultation and health information the patient chooses to provide
• order and payment information, including products, billing details, payment method, transaction reference, gift-card code and refund status
• technical information, including IP address, browser, device, cookies, security logs and website activity
• communication and marketing preferences, messages, reviews and support requests

Why we use information

U-Prime uses personal information to:

• process orders, deliveries, payments, refunds and gift-card balances
• arrange and administer online consultations
• provide relevant booking information to the assigned healthcare provider
• send confirmations, reminders, rescheduling notices, cancellation notices and service messages
• respond to enquiries and provide customer support
• secure the website, prevent misuse and maintain operational records
• meet clinical, accounting, consumer protection and other legal obligations
• send marketing communications where the person has requested or agreed to receive them

U-Prime will not use patient health information for general advertising without clear permission.

How we share information

Patient information is restricted to authorised personnel and the healthcare provider assigned to the patient’s care according to their roles. U-Prime may also share the minimum information required with service providers that support website hosting, booking, payment processing, email, messaging, security and business administration. Information may be disclosed where the law, a court or a competent authority requires it. U-Prime does not sell patient or customer personal information.

Payments

Payment information may be processed by the payment provider selected at checkout. U-Prime may receive the payment status, transaction reference and information required to reconcile the order or appointment. Each payment provider applies its own security and privacy terms.

Retention

U-Prime keeps personal information only for as long as reasonably required for the purpose for which it was collected and for applicable clinical, tax, accounting, fraud-prevention and legal requirements. Retention periods may differ by record type. Information that is no longer required is deleted, anonymised or securely archived where practicable.

Security

U-Prime uses administrative, technical and organisational controls intended to protect personal information against unauthorised access, loss, alteration or disclosure. No internet system can guarantee absolute security. A person who believes that personal information has been misused should contact U-Prime promptly.

Children

A parent or legal guardian should complete a booking for a patient who is under 18 and participate where the healthcare provider requires it. U-Prime may ask for information needed to confirm the guardian’s authority.

Choices and requests

Subject to applicable law, a person may ask U-Prime to provide access to personal information, correct inaccurate information, explain how it is used, withdraw consent or consider a request for deletion or restriction. Some information may need to be retained for clinical, transaction, security or legal reasons.

Cookies and analytics

U-Prime may use cookies and similar technologies for website functions, security, preferences, shopping activity and measurement. Browser controls can restrict optional cookies, but some website functions may not work correctly without essential cookies.
